FAMILY INCOME FORM

All schools are required to distribute and collect the Family Income Inquiry Form for the 2023-2024 school year in order to qualify for Title I status. This brings additional funding to our school.

Although meals are served at no charge to students, families are asked to complete Family Income Inquiry Forms. Please click on the link below to complete the form online.

What you will need to provide during the application process:

  • The names and incomes of every member of your household
  • School, grade, birthdate of every student in your household (Optional)
  • The last 4 digit of your social security number and electronic signature
  • A valid email address or phone number for the Office of School Food to communication about the status of the application. (Optional)
